August - Ask Me Anything - Please Read
Looks like Monday mornings are not a good option for Live "Ask Me Anything" No recording for meeting today Please let me know in the comments if Live "Ask Me Anything" are valuable The attendance has been low the last couple meetings If there is a better date and time for the majority of people we will work on using that going forward If you would prefer another format other than Live meetings then please make suggestions Thanks Michael Twyman

The time consideration often is a challenge for me. I am currently traveling between Pa and Maine to visit with the grandchildren. I absolutely love the opportunity to ask and receive thoughtful responses to ongoing questions and to learn from others. I most often listen to the recording and read the succinct narrative you provide. I know that puts more on the live participates who attend. I also think that redundancy for me is essential. Thank you for all who attend and provide a rich environment for learning.
SLCO1B1
I need to dig deep on my SLC1B1 gene for a minute. I know there’s not much evidence that Ezetimibe would could musculoskeletal issues but I have developed severe and quite debilitating plantar fasciitis. When looking back at my notes it began about 8 weeks after starting Ezetimibe. The data are convincing for Statins of course. But my notes also show that I stopped red yeast rice a week after starting due to arthritic finger pain which resolved upon discontinuation. I’m beginning to wonder if lipid lowering agents and I can’t coincide.

Ask Me Anything - July 2026
Heart Attack Proof - Skool Community (Ask Me Anything) Fri, 10 Jul 26 ApoB vs. LDL-C for Lipid Management - ApoB measures lipoprotein particle count; LDL-C measures cholesterol concentration - Analogy: cholesterol is the cargo, lipoprotein is the ship, ApoB is the protein on the hull - ApoB tracks cardiovascular risk better than LDL-C - Especially in insulin-resistant or pre-diabetic patients (discordant cases) - Target cut points: ApoB 70 or 55 mg/dL - Treatment selection guided by cholesterol balance test (Boston Heart) - Hyper-producer: statins or bempedoic acid preferred - Hyper-absorber: ezetimibe preferred Top 5 Inflammatory Markers for Cardiovascular Assessment - hsCRP - Lp-PLA2 activity - Myeloperoxidase - Oxidized LDL or oxidized phospholipids - ApoB - Bonus: GGT (high GGT signals low liver glutathione, more oxidative stress) Blood Pressure: Causes, Fluctuations, and Monitoring - Elevated BP is a maladaptive response to environmental stressors - Toxins, heavy metals, air pollution, insulin resistance, low nitric oxide - Long-standing high BP increases arterial shear stress and damage - BP rises with illness/pain due to catecholamines and inflammatory cytokines - Short-term BP lowering options: - Isometric exercise (wall squats), Zona Plus grip device, RESPeRATE breathing device - Gold standard monitoring: ambulatory 24-hour BP monitor (BioBeat patch or Welsh Allyn cuff) - Key patterns to watch: morning cortisol spike, daytime stress response, nocturnal dipping - Consumer wearables not yet FDA-approved for continuous BP in the US - Helo band available in Canada/Europe; Oura Gen 5 tracking BP trends via pulse wave velocity - FDA will likely limit Helo to spot checks only when it enters the US market CT Imaging, AI Plaque Analysis, and Cardiac Risk - Three CT imaging tiers for asymptomatic patients: 1. CAC score: low cost, low radiation; zero score = low 10-year risk; misses soft plaque 2. CT coronary angiogram: shows stenosis and soft plaque; now at least on par with stress tests 3. CT angiogram + AI plaque analysis: quantifies total and non-calcified plaque volume

Thank you very much for addressing my questions so succinctly. I have read the narrative as well. I always enjoy hearing about your personal experiences and how you’ve arrived at your current practice. I am now a firm believer in “the numbers don’t lie” and have been following them more specifically and seriously. i’ve learned a great deal from the AMA’s and posted videos. I’m very grateful for the work that you do and the education you provide!.The information helps guide a more informative and boarder decision making.
