⏱️ TIRZEPATIDE: HOW DOSING AND TIMING WORK IN THE TRIALS Tirzepatide is the compound in Mounjaro and Zepbound. Like semaglutide, it has large human trials behind its schedule. Here's how that schedule works and why it's set up that way. WHAT IT IS Tirzepatide works on two gut-hormone signals: GLP-1 and GIP. That's why it's called a "dual agonist." It's FDA-approved as Mounjaro for type 2 diabetes and as Zepbound for weight management and for obstructive sleep apnea in adults with obesity. WHAT PEOPLE USE IT FOR • Weight loss • Blood sugar control in type 2 diabetes • Sleep apnea linked to obesity WHY ONCE A WEEK Tirzepatide has a half-life of about 5 days, meaning it takes about 5 days for half of it to clear. That long action is why it's given as a once-weekly injection. WHY IT STEPS UP SLOWLY In the trials, everyone started at the same low amount and moved up in steps about every 4 weeks. The main reason was to reduce stomach side effects like nausea, diarrhea, and vomiting. Not everyone moves to the highest step. The labels give a range of maintenance levels, and the prescriber picks one based on response and tolerance. WHAT THE RESEARCH ACTUALLY SHOWS • In a large trial of adults with obesity (SURMOUNT-1), people on the highest dose lost about 21% of their body weight on average over 72 weeks, compared with about 3% on placebo. • In a head-to-head trial, tirzepatide led to more average weight loss than semaglutide (about 20% vs about 14%). • Evidence level: Strong. These are large human trials. COMMON QUESTIONS Does the day of the week matter? Trials used the same day each week to keep levels steady. The labels explain how to handle a missed dose or a change of day. Does it interact with birth control pills? The label notes that tirzepatide can make oral birth control less reliable for a while after starting and after each step up, so this is worth raising with a prescriber. What about "research" tirzepatide? Research-vial versions aren't the approved product. The purity, amount, and dosing accuracy of those vials were never tested in the trials.
