Hi doc, Unfortunately I'm also not able to make the live call. do have a question tho, related to the dose increase which I've recently initiated (180mg/wk > 400mg/wk): I've increased my dose in order to get to those optimal levels of 2000-3000 ng/dl total T. Started 8 days ago, and I noticed two things: 1. my sleep was terrible for the first 5 days but it seems to get better now. 2. My weight increased from 88.5kg to 92.1kg (7 lbs) already and I look a bit bloated. Noteworthy: als started implementing your hydration protocol, drinking salted water. My question is: are these effects common and to be expected? Will they subside over time? What is the mechanism behind it? Tnx in advance!

250mg every 3 days
Hi dr Ty, I’ve been delving into your content (good stuff!) and I noticed that you advocate for quite a large TRT dose of 2500mg per month in order to get levels between 2000 - 3000 ng/dl. That’s a quite unique approach which honestly intrigues me. I’ve read studies supporting the fact that 600mg test weekly is tolerated quite well in men, but you hardly ever see this recommended by any TRT professional. Some would even state this to be a body building cycle. I would love to learn more about your views in this matter. What brought you to this protocol and why is it superior In your view (contrary to other protocols for example 200mg per week)? How do you manage overall health and androgen fatigue on these doses? Thanks in advance for elaborating.

@Ty Vincent interesting. just watched the video. Is there a recommended approach to get to these dosages, or can one just start with 250mg every 3 days out of the gate? I'm asking because when i started TRT myself, 150 a week, was a total shock to my body and I had to get used to it. Currently at 250mg a week, and doing well, but lacking the mental benefits which you've mentioned in the video (doing the stuff I used to enjoy doing, etc) which is why I intent to increase and see what this brings me.

@Ty Vincent I will start implementing this and see in 3-4 weeks how things play out. I'll run a bloodtest to see where my levels and health are at. I do wonder tho, if there are other risks associated with these "higher than commonly heard of" TRT levels. For example, won't left ventricular hypertrophy and cardiomyocyte apoptosis be a bigger concern when androgen levels are elevated all the time? I'm not trying to second guess everything you state doc, but I have only one health so I try to be diligent when it comes to big factors influencing it. Thanks in advance for addressing these concerns.
Doctor Says Don't Use Anastrozole – Do You Agree?
Hi Dr. Tyler Vincent, I started TRT about 8 weeks ago. My latest blood work shows a total testosterone of around 1,450 ng/dL. My combined estrogen (E1 + E2) is approximately 152 pg/mL (Estrone 86 pg/mL + Estradiol ~66 pg/mL), so I'm above the range of 50–100 pg/mL that I've often seen discussed. I brought this up with my TRT doctor. He told me that this isn't necessarily a problem because estrogen is expected to increase proportionally with testosterone. He said that if I want to lower my estrogen, I should simply reduce my testosterone dose. He also said that if I decide to use anastrozole, it would be at my own risk, and he would not recommend it because lowering estrogen could disrupt the testosterone-to-estrogen balance and potentially lead to side effects such as reduced libido, depression, and other symptoms. My question is: Do you agree with this approach? In someone with a testosterone level of around 1,450 ng/dL, do you think estrogen should simply be accepted as long as it rises proportionally, or do you believe there is an upper level (for E2 or combined E1 + E2) where using a low-dose aromatase inhibitor becomes beneficial, even if testosterone remains high?

@Ty Vincent well explained. I was wondering if there is a rule of thumb that can be applied when it comes to dosing Anastrozole (at least initially)? I realise it highly depends on the person, and blood work is essential here, but if there is an optimal starting dose of testosterone (250mg every three days for example) which gets 80% of men where they need to be, there must be ball parks for how to dose an AI as well? Don't know if it's bro science but I've heard that 1mg/wk anastrozole per 250mg/wk of testosterone is a decent start. What is your view on this, and I guess the real question is: how do you approach dosing an AI?

The hematocrit debate..
Hi Dr Ty, I’ve been on TRT for 2 years now and have been feeling great. I take good care of my health and sleep, nutrition and exercise are a priority for me. Just watched one of your videos on YouTube called TRT myths. The one thing that my doctor keeps zooming in on is my elevated hematocrit of 58%. He states that this is way too high and can be dangerous. I was somewhat confused when watching your video, where you state that there is no evidence for this to be an issue and that a high hematocrit is of no concern. I very much would like to read up on this subject in order to understand it better and to figure out for myself what science actually states. Could you point me in the right direction with regards to scientific papers and literature? Thanks in advance for taking the time to answer my question.
