Client is post-partum mom with a newborn to 4 month old who is not eating well with possible tongue tie or just had a TT release and is uncoordinated with suck-swallow-breath sequence. Feeding is slow, painful, frustrating and gaining weight slowly. Babies also have an undiagnosed neck torticollis with postural asymmetries that complicate and make feeding even more difficult. Pains & Frustrations: 1. Often breastfeeding takes 45-60-75 minutes and is exhausting to mother and infant, takes so long that it there is very little time in between feedings for rest or engagement. 2. Mother can typically have painful breast with blisters/blebs, engorgement, sore/cracked nipples, mastitis, plugged ducts. Breastfeeding is painful, time consuming and not rewarding. 3. Baby is underweight and this creates anxiety and fear of "malnutrition" which is really undernourishment and need for more frequent feeds. 4. If not yet with a diagnosis of a tongue tie, mom/baby make multiple trips to various practitioners (Pediatrician, lactation consultant, doula, OT/PT, dentist, ENT) to find out why the baby is not eating well and/or gaining weight. 5. Inefficient feeding and feeding routine is paramount and consumes the family focus and time. Difficult and stressful if there are more children in the family Worries and Frustrations: 1. Baby is underweight and may need to go to solely bottle feeding or at the worst case need an NG (nasal gastric) tube for nutrition. 2. If they have the tongue tie released , feeding may not get any better or get minimally better or even worse. 3. Mom's milk supply might be compromised as the baby cannot stimulate milk production by their lack of a strong and effective suck. 4. Mom may need to triple feed (breastfeed, pump & bottle feed) just to adequately feed the baby and provide the required daily fluid nutrients needed. 5. Difficulty getting in a routine or on a productive feeding schedule bc each feeding takes so long. Can lead to inefficient sleeping.
