For years, the dominant thinking has been that dementia and Alzheimer's are, for some people, simply an inevitable result of getting older with the wrong genes. Recent research tells a different story, and a new prospective cohort study in The Journal of Prevention of Alzheimer's Disease is one of the latest to confirm it. Researchers followed 494 cognitively unimpaired adults for an average of 3.9 years with repeated MRI and PET scans, tracking the three pathologies that underlie most dementia: white matter damage, amyloid, and tau. Higher blood pressure, high cholesterol, heart disease, smoking, and lower education all predicted faster accumulation of white matter damage, the hallmark of the vascular disease that contributes to a large share of cognitive decline. The APOE ε4 gene, by contrast, mainly influenced amyloid and tau. Since an estimated 45 percent of dementia cases are attributable to modifiable risk factors, this study helps explain where that leverage lives: largely in the health of our blood vessels. In other words, genes load the gun, but environment pulls the trigger. We have an enormous influence over our cognitive health as we age, through unglamorous fundamentals like blood pressure, metabolic health, movement, and not smoking.
