👋 Welcome to Psych Mastery With Dr. Mena
First of all—welcome! I'm glad you're here. If you're a PMHNP student, a new graduate, or a practicing psychiatric provider looking to strengthen your clinical skills, you're in the right place. Most training programs teach you how to pass exams, but they don't always prepare you for the complexity of real-world psychiatric practice. That's why I created this community. Here, we'll focus on practical psychiatry that you can apply immediately in clinical practice, including: 🧠 Diagnostic reasoning and differential diagnosis 💊 Psychopharmacology made practical 📋 Real-world clinical cases 📚 Evidence-based psychiatric education 💡 Clinical pearls you can use with your next patient 🤝 A supportive community where you can ask questions, learn, and grow alongside other PMHNPs My goal is simple: to help you become a more confident, thoughtful, and skilled psychiatric clinician. — Dr. Mena Mirhom, MD, FAPA Board-Certified, Columbia-Trained Psychiatrist

Reflection & Discussion 💭
What Changed in Your Thinking This Week? This week, we talked about something that sits at the center of good psychiatric practice: Before rushing toward an answer, learn to ask better questions. Clinical confidence doesn't mean knowing everything immediately. Sometimes, confidence means being comfortable enough to pause and say: What am I missing? Take a moment to reflect: What's one thing you learned—or one question you started thinking differently about—this week? It can be something from this community, your clinical experience, or your own studies. 👇 Share your reflection below.

Ask Dr. Mirhom 🎙️
What’s Your Clinical Question? Psychiatry is full of situations where the answer isn’t always straightforward. Sometimes the most valuable learning comes from taking a question you’ve encountered in practice and slowing down to really think through it. So, I want to hear from you. What is one question you’ve been wanting to ask about psychiatry or clinical practice? It could be something related to: - 🧠 Clinical reasoning and decision-making - 🔎 Differential diagnosis - 💊 Medication management - 💬 Navigating difficult patient conversations - 🎯 Building confidence as a clinician - 📚 PMHNP education and training - 🩺 Approaching challenging or complex clinical presentations - ⚖️ Managing uncertainty in clinical practice It doesn’t have to be a perfect or highly technical question. If you’ve wondered about it, there’s a good chance someone else in the community has too. 👇 Drop your question in the comments. I’ll be looking through the questions and may select some for future discussions, educational posts, or videos. Let’s use this community to learn from the questions that come up in real clinical practice.

Case Discussion 🩺
What Would You Do? A 32-year-old patient comes to your outpatient clinic reporting worsening anxiety and insomnia. They say: “My mind won't shut off. I'm sleeping only 3–4 hours a night, but strangely, I don't feel tired.” They also report increased energy and productivity at work. Before jumping to a diagnosis or medication, what would you want to know next? Think about: 🧠 Mood history 😴 Changes in sleep patterns 💊 Current medications 🍷 Substance use 👨‍👩‍👧 Family psychiatric history 📅 Timeline of symptoms There is no need to have the “perfect answer.” The goal is to practice your clinical thinking. 💬 What questions would you ask next?
