Hospital discharge info
Apologies for all of the questions this week, but we really value your input. We are working on a hospital information letter which would be tailored to each stroke survivor with key information. I've attached drafts of 2 possible documents which we might use. One was designed by us and the other is from the World Stroke Organisation. Please could you provide feedback on both documents to help guide us in producing a final document which will hopefully be the best of both worlds.

Hi first letter, contains the kind of information I mostly did not learn until after I was home for a month (5 +months after the stroke). Also for areas I did recognise, I would had liked know results of my 24hr monitor, and echocardiogram. It says it is aimed for heathcare professionals to explain to patients and families, but the kind of information but we don’t just heed a checklist, we need to understand results and consequences. it’s doesn’t seem to be really aimed at me and really are things my hospital should have told me more about. E.g. the 24hr monitor showed I had afib but was that a significant thing, how bad, I’m sure there must be degree?. Plus it needs a glossary if we are the intended audience - the terms and abbreviations are mostly meaningless On the second patient form I found it a little confusing, at first I thought I was telling me to sing. Also when you are bedbound telling you to aim for exercise just adds to the already high level of depression you have when you come home. So it needs to be more tayored to the individual. Overall i welcome anything that increases my understanding of my situation
